Supervisor name within project* Name of SEE Project Director or Deputy Director who will sign contract on behalf of the project.
Title of service* Business service being provided, i.e. Web Design, Grant Writer, Consultant, etc.
Start date of work* Must be a future date, work cannot commence until contract is complete.
Compensation rate* Hourly, monthly, or total to be paid.
Compensation schedule* Frequency of payments, i.e. monthly, two payments, lump sum, etc.
Total contract value* Total amount to be paid over the course of the contract. Not to exceed this amount.
Evidence of Independent Contractor status* Accepted forms of evidence include business name, website URL, copy of a business card.
Description of work being performed* Including key areas of responsibility.
